The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

Post 1 - 30 hours per week:

Thursday 8am - 2pm

Post 2 - 27.5 hours per week:

Wednesday 7.45am - 6pm

Urgent Care Practitioners

South Hambleton and Ryedale Primary Care Network stretches across 7 practices in North Yorkshire.  Our successful PCN was a finalist for the PCN of the year award and, in addition to this, 5 of our 7 practices recently topped the latest patient survey poll for our CCG, with patient satisfaction scores reaching as high as 98%.  We enjoy established strong working relationships between practices and wider health and community networks.

SHaR PCN is establishing a local urgent primary care service to enhance patient care and support our established happy and enthusiastic primary health care team. We aim to recruit a team of urgent care practitioners to provide an efficient integrated urgent care service, managing undifferentiated primary care patients who are judged to have urgent same day need.  Working independently via telephone, through face to face consultations and providing a home visiting service, the practitioner will be accountable for delivering high quality, safe, clinical decision-making and expert patient care. Acting within professional boundaries, duties will cover all of the patient journey, including:  history taking; examination; holistic clinical assessment; diagnosis; investigation; treatment; supply of medication via PGD’s or utilising a prescribing qualification; valuation of patient care’ and safe handover and communication. An integral member of wider multidisciplinary teams, the practitioner will help formulate and work to advance care plans, together with members of the primary health care team and attached health, social and community services, including paramedics and secondary care colleagues.

To ensure that we achieve this goal we look to recruit candidates of a high calibre that share our passion for delivering excellent patient care.

As such, the ideal candidate will have a demonstrable track record in building relationships, a strong team work ethic but also has an ability to work autonomously.

Although this is a full-time position (37.5 hours per week), job share will be considered.
